Checklist Items

New Accounts  
  • One form of a current government-issued photo identification. Photo identification must include:
    • Picture
    • Signature
    • Issue Date
    • Expiration Date
  • Minors, 15 – 17 years old, are required to present one form of a current government-issued photo identification. Photo identification must include:
    • Picture
    • Signature
    • Issue Date
    • Expiration Date
  • Minors, 14 years old and under, are required to present a Social Security card or Military ID.

 

  • NOTE: If ID has been issued within the past 60 days (less than 60 days), an additional form of photo identification must be provided.
  • NOTE: If address on ID is incorrect, utility bill (ex. electric, water, sewage, cable, or landline phone) less than 60 days old with correct address must also be provided. Or a pay stub or Social Security statement or check (not a Social security card) within the last 60 days which include the name and address for the individual presenting the ID.This does not apply to individuals 17 and under.
 Account Changes  
  • Name Changes
    • Updated photo ID (reflecting name change)
    • Original or certified true copy of legal document authorizing name change (ex. marriage license, divorce decree, court order, etc.)
  • SSN Changes
    • One form of identification to verify SSN (ex. Social Security Card)
Loans
  • One form of a current government-issued photo identification
  • Optional documentation may include:
    • Vehicle registration
    • Vehicle title
    • Payoff information
    • Income verification via paystub

Business Appointments

All business entities will be required to provide information and documentation for the controlling party and for all owners of the business. A controlling party is an individual with significant responsibility for managing the legal entity.

Information required:

  • Full name
  • Address
  • Date of birth
  • Social Security number
  • Phone number
  • Driver’s license number
  • Driver’s license state
  • Percentage of ownership, if applicable
  • Primary occupation, if also an owner of the business

Sole Proprietorship Current picture ID and one of the following:
  • Business License or Retail License
  • Previous or current year tax return with Schedule C

Partnership Current picture ID and two of the following:
  • Business License or Retail License
  • Partnership or Operating Agreement
  • Previous or current year tax return with Schedule C
  • Certificate of Existence/Certificate of Good Standing
  • IRS Letter 147c or IRS Form SS-4
Corporation or LLC Current picture ID and two of the following:
  • Business License or Retail License
  • Certificate of Existence/Certificate of Good Standing
  • Articles of Incorporation or Organization
  • Previous or current year tax return with Schedule C
  • Operating Agreement
  • IRS Letter 147c or IRS Form SS-4
Organization or Association Current picture ID and two of the following:
  • Copy of minutes appointing officers
  • Articles of Incorporation or Organization
  • Letter from secretary authorizing signers
  • IRS Letter 147c or IRS Form SS-4
